Below are the two forms you will need if you wish to pay your REPs registration by monthly installments. Please download and print all three.

• • Insurance form for re-registration
    This is a form for PT’s contractors and people who want additional insurance. All other re-registration forms are sent directly by REPs. Please contact us if you have not received these
  • Medical Incapacity/Maternity Freezing Form
    This form is to place your REPs Registration on hold while you are on maternity leave or a medically incapacitated.

For Education Providers

• • Application to have a course recognised by REPs (to meet the standard needed to initially register)
